For a patient to be considered for the HealthCare Access program, he/she must be referred by a physician or medical provider and, in need of primary care, diagnostics and/or specialty care.  The person/family must have an income within 200% of the Federal Poverty Guidelines, not have medical coverage of any kind, and be a resident of Forsyth, Stokes, Davie, Davidson or Yadkin county (for at least 90 days).  At the time of the screening appointment, each person must provide:


·         verification of income – three recent paycheck stubs or a verification of employment form completed by the employer, if unemployed then information as to how a person is meeting his/her living expenses

·         copy of most recent taxes filed

·         if self-employed – copy of the accounting books for the last six months

·         if the employer offers insurance, written information on the cost to the employee

·         proof that he/she resides in one of the three counties – NC Driver’s license or state issued ID

·         any support received from family members or other agencies – signed letter from family member(s), letter from agency(s) must be on letterhead

·         information on disability status or food stamps, if applicable

·         a list of all monthly expenses – including a copy of rent/mortgage payment, utility bills, telephones, car payment, credit cards and other monthly expenses 

·         a statement of any savings, pensions or IRAs.


After completing the screening, if the patient is found to be eligible for the program, he/she signs a Responsibilities Agreement and Acknowledgement Form, outlining the rules of the program.  The enrolled patient is given a copy of these documents and an enrollment card, which must be presented every time he/she receives services with the program.  Enrolled patients are recertified every 6 or 12 months depending on eligibility status.

The success of the program is attributed to the support of the physicians, the three local hospitals and other medical providers. The medical community of Forsyth, Stokes, Davie, Davidson and Yadkin counties is very generous in caring for the medically uninsured.  HealthCare Access/Forsyth HealthCare, Inc. is a 501c3 organization with an eleven member board with broad community representation.
